If you have insurance you must have:
1. Insurance name & phone number
2. Subscriber date of birth
3. Subscriber social security number

What to do before your appointment
Take your regular medications as you normally do.
Take any necessary premedications 1hr before
Eat as you normally do.
If you request nitrous oxide, don't eat before
appointment.
